Welcome to the TriWest Provider Directory!
This help file will assist you with using the directory to find network providers.
The page above is the Provider Directory Home Page. From here, the following options are available:
Provider SearchesBy Location - find a provider within a specific city/state or within a specified distance from a zip code.
By Name - find a provider by a portion or all of the provider's last name.
By Group Name - find a group practice by a portion or all of the group's name.
By Facility - find medical facilities or suppliers, such as hospitals, surgery centers, durable medical equipment vendors, etc.
By Specialty - find a provider or facility by specialty or facility type within a specific city/state or within a specified distance from a zip code.
By Island - find a provider on a specific Hawiian Island.

The Provider Directory home page is readily available
throughout your search.

link at the top right of your screen.

Searching for a Provider by Location
From the homepage, select Search by Location. The following page will be displayed:
This page provides access to providers by geographic location. There are two basic types of searches:
1. Search by Zip Code
This is the most accurate search type.
To use this feature, under "Search Criteria", select the following:
. Click "Search by Zip Code".
. Select a radius value from the dropdown menu labeled "Location: within ____
miles". Values can be selected from 10 to 250 miles (required).
. Enter the desired 5-digit zip code (required). If you do not know the
correct zip code, click the
Find Zip Code link. This will direct you to the U.S. Postal Service zip
code search website.
. Select a specialty type from the "Specialty" dropdown menu (required).
If you need more information on specialty types, click the
What do these Specialties mean?
link. A list of specialty definitions will be displayed.
You may optionally complete the information on the right side of the screen, under "Optional Information". Use this feature if you want to limit your search to providers with a specific last name, gender or language spoken. If you choose to search by last name, you can enter as few as two of the first characters of the last name (for example, enter "TU" or "TUC" for "Tucker"). If you know the exact spelling, however, you will receive the most correct results by entering the entire last name.
Once all information has been entered, click the 
button. Your search criteria will remain displayed, as well as any matching
providers.
Search Results: Up to 250 providers can be displayed in the Search results portion of your screen. Use the small vertical scrollbar located within the results window to see more results.
The provider's Address and phone number are displayed, as well as the Distance calculated from the Zip Code you entered. Closest providers are listed first.
Details about each provider can be viewed by clicking the underlined provider name.
Directions can be accessed by clicking the
icon. This will direct you to a mapping program for maps
and driving directions.
The provider's Website can be viewed by clicking the 
icon. NOTE: Not all providers have websites.
The listing can be sorted by clicking the underlined column headings for Name or Distance.
Printing the search results: Two printing options are provided:
. Printer-friendly web page - access this feature by
clicking the 
tab
located at the top right of the search results.
. Adobe PDF file - this feature allows you to print a listing of providers, or to save the listing to your own computer for future reference.

2. Search by City and State
This will find only those providers whose addresses contain the city and state
specified. If you are searching for a provider in a large metropolitan area, the
Zip Code search may provide more accurate results. To use this feature,
under "Search Criteria", select the following:
. Click "Search by City/State".
. Enter the city name - you may enter a pritial city name. Correct spelling is
important!
. Select the state from the dropdown list.
. Select a specialty type from the "Specialty" dropdown menu. If you need more
information on specialty types, click the
What do these Specialties mean? link. A list of specialty definitions
will be displayed.

Searching for a Provider by Name

To quickly search by last name, enter the following
- At least the first 2 characters of the last name (required)
- The provider's state (required)
- At least the first 2 characters of the first name (optional)
- The provider's city (optional)
- Enter the provider's 5-digit Zip Code (optional). If you do not know the correct Zip Code, click the Find Zip Code link. This will direct you to the U.S. Postal Service zip code search website.
You may optionally complete the information on the right side of the screen, under "Optional Information". Searching for a Provider Group by Name
From the homepage, select Search by Group The following page will be displayed:
This page provides access to practice groups by group name.
To quickly search by group name, enter the following
- At least the first 2 characters of the group name (required)
- The provider group's state (required)

providers.

The group's Name, address and phone number are displayed.

Searching for a Facility
This page provides access to facilities such as hospitals and laboratories. From the homepage, select Search by Facility. The following page will be displayed:
To quickly search for a facility, enter at least one of the following:
- At least the first 2 characters of the facility name
- A facility type
AND
- The facility's state (required)
. You may optionally complete the information on the right side of the screen, under "Optional Information". Use this feature if you want to limit your search to providers within a specific geographic location. You may search within a specific distance of a zip code, or by a city name. If entering a city name, please remember that correct spelling is important.
Once all information has been entered, click the 
button. Your search criteria will remain displayed, as well as any matching
providers.
Search Results: Up to 250 facilities can be displayed in the Search results portion of your screen. Use the small vertical scrollbar located within the results window to see more results.
The facility's Name, address and phone number are displayed.
The type of facility is displayed.
Directions can be accessed by clicking the 
icon. This will direct you to a mapping program for maps
and driving directions.
The facility's Website can be viewed by clicking the 
icon. NOTE: Not all facilities have websites.
The listing can be sorted by clicking the underlined column headings for Facility Name or Facility Type.

Searching for a Provider by Specialty
From the homepage, select Search by Specialty. The following page will be displayed:
This page provides access to providers and facilities by geographic location. There are two basic types of searches:

Accessing the Statewide Directories
From the homepage, select Statewide Directories. The following page will be displayed:
- Select the desired state from the dropdown list.
- Select a format. Available formats are
- Printer-friendly web page (can be printed using the "Print" feature of your web browser)
- PDF (can be printed immediately or saved to your computer's hard drive)
NOTE: Some of the directories are very large and may take additional time to download.
Accessing the Prime Service Area (PSA) Directories
From the homepage, select Prime Service Area Directories. The following page will be displayed:
- Select the desired Prime Service Area from the dropdown list.
- Select a format. Available formats are
- Printer-friendly web page (can be printed using the "Print" feature of your web browser)
- PDF (can be printed immediately or saved to your computer's hard drive)
NOTE: Some of the directories are very large and may take additional time to download.
